Abstract
In traffic accident, a high index of suspicion is needed for early diagnosis of ipsilateral knee ligament injury in a patient who has femur, tibia or fibula fracture. When the diagnosis and treatment of Ipsilateral knee ligament injuries was delayed, the patients have patients have complained of knee joint instability, limitation of knee motion and pain.
We retrospectively analyzed the incidence and diagnostic time of the Ipsilateral knee ligament injuries of 345 cases admitted because of the fracture of femur, tibia or fibula.
@ES The result are summerized as below.
@EN 1. Of 345 cases, the total number of the ipsilateral knee ligament injuries were 13 cases93.8%) : 5 cases of 171 cases(2.9%) in tibia fracture, 4 cases of 126 cases93.4%) in femur fracture, 2 cases of 16 cases(6.3%) in only fibula fracture, 1
case
of 22 cases in both femur and tibia fracture(4.5%), and 1 case of 6 cases in both femur and fibula fracture(16.7%).
2. Diagnosis was delayed for an average 30 weeks and only seven of thirteen injuries of the knee ligament were diasgnosed during the initial stay in the hospital.
3. the associated ipsilateral knee ligament injuries cnsisted of posterior cruciate ligament ijury in five cases, anterior cruciate ligament injury in one case, lateral collateral ligament injury in one case, patellar tendon injury in one case
one
case
and combined ligament injury in five cases.
4. For the associated knee ligament injuries surgical interventions were done in six cases and conservative treatment in seven cases.
There were significant complication in three of four cases which diagnosis of knee ligament injury was delayed more than 1 year.
5. The associated ipsilateral knee ligament injuries were most common in active young age group : 10 cases of 102 cases(9.8%) between twenty and thirty five years old, none in age les than ten years old or more than forty six years old.
6. In conclusion, a high index of suspicion is needed for early diagnosis of the opsilateral knee ligament injury in treatment of femur, tibia, or fibula fracture in order to prevent the complications of the knee ligament injury.
